Job description
Leoforce is seeking a Hospital RN Case Manager in West Nyack, NY to oversee patient care plans across the continuum from admission to discharge. This Full-time role emphasizes collaboration, utilization review, and timely care decisions within a 200-bed academic medical setting.
The position requires 3–5 years of relevant clinical experience and a current NYS RN license, with CCM certification preferred. Will work Monday–Friday 8:30 AM–5:00 PM with rotating weekend coverage.
