Job description
San Antonio Community Hospital in Upland, CA seeks a medical social worker (MSW/LCSW) to identify, screen, assess, and enroll patients into CHIP and provide social service interventions. You will conduct psycho-social assessments, recommend post-hospital care for patients with chronic conditions, and mentor student health coaches.
Requirements include a Master’s in Social Work (MSW or LCSW), California licensure, CPR, and 2–5 years of hospital experience.
