Acute Care Nurse Practitioner St, Louis, MO Full-Time Onsite
We†re looking for a compassionate and experienced Acute Care Nurse Practitioner to join a collaborative care team dedicated to providing high-quality care to older adults. This role is ideal for a provider who enjoys building meaningful patient relationships while delivering timely, comprehensive care.
What You†ll Do Provide primary and acute care, including sick visits, walk-ins, hospital follow-ups, and urgent needs.
Evaluate, diagnose, treat, and document patient care.
Partner closely with physicians, RNs, medical assistants, and other care team members.
Coordinate care with patients†primary care providers to ensure continuity.
Support patient outreach, quality improvement, and clinical initiatives.
Help create a welcoming, efficient, and positive patient experience.
Participate in occasional after-hours, weekend, or holiday coverage as needed.
Travel between nearby locations when required.
What We†re Looking For Active Nurse Practitioner license in the state of employment.
1+ year of experience in primary care, internal medicine, geriatrics, or family medicine.
Current BLS/ACLS certification .
Strong clinical, assessment, diagnostic, and documentation skills.
Experience caring for older adults with complex or multiple medical conditions.
Excellent communication and interpersonal skills.
A compassionate, team-oriented approach and genuine commitment to patient care.
