Job description
Join our growing healthcareteam providing patient-centered primary care to residents in skilled nursingfacilities throughout Mishawaka, Indiana. This part-time position offers aflexible schedule, no call or overnight responsibilities, and the opportunityto work collaboratively with physicians and facility staff to deliverhigh-quality, evidence-based care.

Position Highlights
Part-time with flexiblescheduling
Six-hour facility shifts;rounds typically begin between 7:00 a.m. and 10:00 a.m.
No call or overnight shifts
Average 15-20 patient visitsper day
Conduct daily rounds, manageacute and chronic conditions, prescribe medications, and develop treatmentplans
Collaborate with nursingstaff and interdisciplinary teams
Ensure compliance withclinical and regulatory standards

Requirements:
Active, unrestricted U.S.Nurse Practitioner or Physician Assistant license
Board Certification, NursePractitioners: ANCC, AANPCB, PNCB, or AACN. Physician Assistants: NCCPA
Board Certified or BoardEligible: FM, IM, Geri, PM&R & ED
Excellent clinical judgment,communication, and patient care skills
Ability to workindependently while collaborating effectively within a multidisciplinary team
