Join our growing healthcareteam providing high-quality, patient-centered care to residents in skillednursing facilities throughout Sulphur, Louisiana. We are seekingcompassionate Nurse Practitioners and Physician Assistants who are passionate aboutimproving the health and quality of life of older adults in a collaborative,team-based environment.
Position Highlights:
Full-time opportunities with flexible scheduling
Six-hour facility shifts forfull-time providers (rounds typically begin between 7:00 a.m. and 10:00 a.m.)
No call or overnightresponsibilities
Provide patient-centeredprimary care to residents in skilled nursing facilities
Conduct daily rounds, manageacute and chronic conditions, and prescribe medications and treatment plans
Collaborate with nursingstaff and interdisciplinary care teams
Document efficiently using astreamlined EHR with AI Scribe, voice dictation, and a company-provided iPadwith LTE
Access UpToDate forreal-time clinical guidance

Requirements:
Active, unrestricted NursePractitioner or Physician Assistant license
Board Certification (ANCC,AANPCB, PNCB, AACN, or NCCPA)
Skilled nursing, long-termcare, geriatrics, or post-acute care experience preferred
Ability to workindependently while collaborating with physicians, nursing staff, andinterdisciplinary teams
