Job description
Optum Home & Community Care is hiring HouseCalls Advanced Practice Clinicians (NPs or PAs) to perform annual in-home health assessments for Medicare Advantage members. This field-based, non-prescribing role focuses on education, gap closure, and collaboration with PCPs.
Active certification and licensure are required, along with a valid driver’s license and ability to travel to patient homes. No on-call duties, flexible daytime hours, and a competitive compensation package are provided.
