Job description
What You'll Do:
Provide in-home primary care for chronic disease management, dementia care and preventive healthcare
Perform cognitive screenings, annual wellness visits, follow-ups, and hospital-to-home transition supervision
What You Bring: Willingness to travel to patient's homes
Active Virginia Nurse Practitioner
DEA registration or eligibility
Medicare PECOS enrolled
Experience in primary care and geriatrics
