Job description
Optum Home & Community Care is recruiting an in-home Advanced Practice Clinician (NP or PA) to perform annual health assessments for Medicare Advantage members in patients' homes. The role includes education, gap closure, and collaboration with primary care providers to improve outcomes.
You will address social determinants of health, navigate varied home environments, and participate in a growing, interdisciplinary care team within a supportive, performance-driven program.
