Optum HouseCalls is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health assessments for Medicare Advantage members. The role is field-based across Missouri, focusing on education, gap closure, and collaboration with primary care providers.
