Job description
Human Services Program Specialist - This is a highly responsible and sensitive position that provides case management to clients in the Ryan White Program. This position provides comprehensive medical and non-medical case management services to clients living with HIV/AIDS. This position serves as part of the interdisciplinary Florida Department of Health in St. Johns County (FDOH-SJC) Early Intervention Services (EIS)/Ryan White Program Team. These activities include but are not limited to:Interviewing clients while completing initial and ongoing risk, need, and social assessmentsAll required forms, Eligibility processEvaluating clients', including medical, social and psychological aspects, and developing an individualized Plan of Care (POC)Coordinating and collaborating on going-care, education, and support with EIS Program Team to meet the individual POCCoordinating services by connecting clients with community resources, healthcare providers, rehabilitation programs, or legal services, ensuring continuity of care and timely access to necessary support and servicesAdvocating for clients by helping them navigate complex systems, communicate with professionals, and make informed decisions about their care and/or other mattersMaintaining regular communication with clients and families, track progress, update care plans, and adjust strategies as needed to achieve desired outcomesKeeping detailed records of client interactions, treatment plans, referrals, and outcomes, and may prepare reports for supervisorMaintaining information in a confidential manner and disclosing information only after proper authorization has been obtainedProviding education, counseling and/or making appropriate referrals and linkages to meet client needsAssisting with the coordination of all HIV counseling and testingCompleting HIV testing and counseling in accordance with statutory requirementsConducting home visits and participating in outreach eventsAssists with programmatic activities which include but are not limited to:Compiling, maintaining and preparing data on clients involved in the programProviding necessary reports and data for grant submission/application and/or administrative reportingPerforms other duties as requiredRequired Knowledge, Skills, and Abilities:Knowledge of basic case management concepts and processesKnowledge of interviewing techniquesKnowledge of HIV/AIDS treatment and careSkilled in written and oral communicationSkilled in active listening, empathy, and conflict resolutionAbility to work independently but also work as part of a team when neededAbility to communicate proficiently and effectively with clients and professionalsAbility to utilize MS Office programs and database management systemsAbility to complete data/statistical analysis and scientific methodsAbility to prioritize and organize work assignmentsAbility to operate technical equipmentAbility to work in a fast-paced environmentAIDS Education and Training Center (AETC) sponsored case management training completed within six months of hireLicensure/registration/certification requirements (If applicable, list the appropriate Florida Statute or federal regulation cite): Valid Florida driver license, HIV 500/501 certification within 60 days of hire (Florida Statute 381.004)Other job-related requirements for this position:Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergencyWorking hours: (A) Daily from 8:00 to 5:00 (B) Total hours in workweek 40 (C) Explain any variation in work (split shift, rotation, etc.) This position requires daily in-office or field workMinimum - Must have a valid Florida driver's license.
