99 POSITION SUMMARY :

The Resource Navigator will support operational aspects of the Division of Human Services Hub and ensure service connections throughout the division to the Behavioral Health/Developmental Programs Department, the Area Agency on Aging, Children & Youth Social Services, the Bucks County Drug & Alcohol Commission, Co-Responders and Veterans Affairs. Professional work of a social work nature to help county constituents in need obtain and utilize all available human services resources. Work involves working directly with those in need to ensure needed social services are obtained and human services resource gaps are filled. Work includes the direct application of problem-solving and goal-setting techniques and interacting with agencies that make up the county's service network and Continuum of care.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES :

Addresses all incoming inquiries and needs to The Hub through phone calls, walk-ins, and emails.

Primary responder to incoming calls, e-mails, and in person visits at The Hub M-F, 9am-4pm.

Responds to referrals and requests from community; clients, Agency staff and other human services providers who desire information and access to human service providers and programs. Maintains detailed records of requests and responses.

Locates and evaluates appropriate resources for clients. Provides timely follow-up and provision of light case management.

Provides social service screenings across multiple domains to identify aging, children/youth, mental health/substance use, and community support needs.

Assists with the location of identified resources Identified in social service screening. Completes and/ensures completion of social service referrals to needed resources.

Provide effective crisis response/triage through trauma-informed support and connect client to appropriate crisis centers/resources

Maintains records of clients, case notes, outcomes, and other documentation through electronic database.

Maintains regular contact with individuals when needed to monitor progress, provide ongoing support, and adjust services as needed.

Participates in relevant training as prescribed and orientation to any human service agencies as needed

Attends community outreach events/resource fairs for purpose of community outreach and program development.

Completes regular visits to the Department of Corrections to meet with Bucks County residents soon to be released back into society.

Provides staffing coverage Hub "Pop-Ups" sites (Hub presence at alternate location).

Obtains needed consents/authorizations to refer to needed healthcare resources, when necessary

Collaborates with other County and community human service agencies (e.g. Area Agency on Aging, BDCAC, Children and Youth, NOVA, etc.) as appropriate

Participates in team meetings held by the Division of Human Services and other key partners.

Assists in identifying gaps in social and human services and reports out to county service coordination meetings.

Attends all street outreach and homelessness related meetings to promote coordination of care.

Collaborates with other outreach workers, social service providers, law enforcement agencies, community organizations and government entities to coordinate efforts, share information within privacy and HIPPA laws, and share resources for those experiencing homelessness or any other basic needs.

Becomes certified in SOAR (SSI/ SSDI Outreach, Access, and Recovery, and handles all Hub SOAR cases.

Performs other duties as required.

QUALIFICATIONS REQUIRED:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Bachelor's Degree in sociology, social welfare, social work, psychology, gerontology, criminal justice, or other related social science and three years of professional level experience In the field of social or criminal justice casework or any equivalent combination of experience and training; (Preferred )Master's Degree with three years human service/criminal justice experience including experience with special populations, e.g. individuals with drug and alcohol, geriatric, childhood, and trauma issues, etc.

Knowledge of and ability to implement the principles of a recovery-oriented system of care

Knowledge of, or ability to quickly learn, the organization and structure of the County court and legal systems as they relate to Individuals with social service needs

Knowledge, or ability to quickly learn and maintain current knowledge, of community social service resources

Ability to work largely independently

Ability to plan and organize work, set priorities and manage workload in an effective and timely manner

Ability to adequately express ideas clearly and concisely, orally and in writing

Ability to establish and maintain effective working relationships with individuals, their families, and community resource providers
