Job description
LiveWell Homecare Agency is seeking a Physical Therapist to provide patient-centered care in a hybrid acute/inpatient setting. You will develop treatment plans, deliver skilled therapy, and consider discharge needs and equipment requirements.
Collaboration with patients, families, and healthcare teams, plus education and referrals, are essential. Responsibilities include delegating tasks to assistants, documenting interventions, and conducting assessments and reassessments for physical therapy
