Job description
Hope For Youth, Inc. in Nassau County, NY, is seeking a Case Planner (Social Work) for a full-time hybrid role. You will support at-risk youth and families through in-home visits, life coaching, and case management under the PINS Diversion Program.
The role requires 2+ years of relevant experience and a degree in social work or related field, with flexible hours including evenings and weekends. Mileage reimbursement is available.
