Job description
Vynca Inc. seeks an experienced Clinical Lead Care Manager (CLCM) to join our team in California.
Under the ECM Clinical Manager, you will coordinate with doctors, specialists, pharmacists, and social services to align on the client’s needs and care, manage client cases, and promote wellness and independence. This hybrid role requires travel throughout Sacramento County up to 5 days per week, with residence within 25 miles of the territory.
