Job description
Corewell Health Hospice is seeking a Nurse Case Manager to coordinate and deliver compassionate end-of-life care to patients and their families. You will assess needs, develop care plans, and collaborate with an interdisciplinary team to ensure the highest quality hospice care.
Responsibilities include home and facility visits, symptom management, education, and thorough documentation in compliance with hospice regulations. A Michigan RN license and 2 years of relevant experience are required.
