Job description
COPE Health Solutions is seeking a Medical Director/Supervising Physician to provide clinical leadership and physician oversight for the Care at Home Solutions program in Los Angeles. You will partner with APPs, RNs, LCSWs, Pharmacists, CHWs, and Care Navigators to deliver high-quality, evidence-based care for patients with complex needs, across AWVs, TOC, CCM, and disease management.
