Job description
Monogram Health is seeking Advanced Practice Providers (NP/PA) to deliver high-quality in-home care to patients with multiple chronic conditions. You will join an integrated care team and provide care in patients’ homes and via telehealth, with a value-based, patient-centered model.
Highlights include 3–4 in-home visits per week, 1–2 telehealth visits, no on-call, and 30–90 minute visits. Competitive base salary plus bonuses and mileage reimbursement support professional growth.
