Job description
Little Flower Children and Family Services of New York is seeking a Nurse Case Manager to coordinate healthcare for children in foster care. This full-time hybrid role requires 3 days in office at the flagship location in Brooklyn, NY. Responsibilities include developing care plans, conducting assessments, and managing healthcare resources. Candidates must possess a valid nursing license, a BSN degree, and ideally have experience in the child welfare system. Strong problem-solving skills and bilingual capability in English/Spanish are preferred.
