Job description
Monarch Healthcare is seeking a Nurse Practitioner or Physician Assistant for outpatient primary care in Wilson and Rocky Mount, NC. The role blends two onsite days per location with one remote day, Monday–Friday, 8am–5pm, with occasional home visits.
You will perform assessments, manage chronic conditions, prescribe medications, and coordinate care with behavioral health and community partners in a collaborative, high-quality care model.
