Job description
Sentara Health Plans in Central Virginia is seeking a Behavioral Health Whole Person Care Coordinator/Registered Nurse (RN) for a hybrid schedule based in Richmond, VA. You will support medically and behaviorally complex members, including children, adolescents, and adults, with care coordination and community-based visits.
The role requires a Bachelor's degree and a state-licensed clinical credential (LCSW, LPC, LMFT, or RN) with 2 years of behavioral health and 1 year of case management
