Job description
Commonwealth Care Alliance Inc. is seeking a Mobile Integrated Care Clinical Manager in Massachusetts. The role focuses on monitoring, follow-up, and clinical care management for dually-eligible enrollees with complex needs, coordinating services and resources to improve health outcomes and quality of life.
