Job description
CareSource in Massachusetts is hiring a Hybrid RN Care Manager to provide care coordination for dually eligible enrollees with complex medical, behavioral, and social needs in Metro West and Central MA.
You will lead the interdisciplinary care team, develop holistic care plans, engage members in homes or community settings, coordinate with providers and community resources, and educate families about benefits and chronic condition management.
