Job description
Western Reserve Area Agency on Aging (WRAAA) in Ohio is seeking an RN Case Manager to support seniors enrolled in MyCare Ohio CareSource waiver programs. The role combines clinical assessment, care coordination, advocacy, and ongoing case management in a hybrid field-based setting.
