Job description
Activate Care is seeking a Registered Nurse for a full-time, hybrid role delivering in-home clinical assessments across Massachusetts. The RN will use clinical judgment to document SDOH needs, coordinate with care teams, and ensure compliance with MassHealth/Medi-Cal guidelines.
Telehealth may supplement in-person visits as needed. The role requires MA RN license, 1 year of clinical experience, EMR proficiency, and willingness to travel.
