Job description
1st Choice is seeking a Coordinated Assessment System (CAS) Assessor to support a statewide initiative in New York. You will conduct person-centered assessments using CAS tools to guide individualized care plans, traveling across regions and working in homes and community settings.
The role offers flexible hybrid work, with in-person assessments and remote documentation, and requires a Bachelor's in human services plus at least 1 year of IDD experience. Bilingual English/Spanish preferred.
