Job description
The CARIN Alliance seeks a Nurse Practitioner to provide primary care to adults on Medicare in underserved communities in Peoria, IL. You will perform comprehensive assessments, manage chronic conditions, and coordinate referrals within a collaborative, patient-centered setting.
You will document thoroughly in the EHR, collaborate with physicians and care teams, and contribute to quality improvement initiatives that enhance access and outcomes for vulnerable populations.
