Job description
Premera is seeking Case Managers (RN, RD, Licensed MH Clinicians) to improve members’ health outcomes. You will engage with members and clinicians, develop collaborative care plans, and use evidence-based skills to promote behavior change and better care coordination. The role includes post-discharge follow-up and contributing to reduced ED visits and readmissions.
