Job description
Premise Health is seeking a PRN Collaborative Practice Pharmacist to provide direct patient care at our Health Center in Columbus, IN. You will improve outcomes through medication management, reconciliation, and patient education within a collaborative care model.
Expect to perform comprehensive reviews, coordinate with providers, and contribute to preventive services while maintaining licensure and pursuing relevant certifications.
