Job description
Hill Country Preferred Senior Care in Mason, TX is seeking a Home Health RN/LVN Case Manager to join our team. Provide skilled nursing care, coordinate patient plans, and ensure timely documentation and communication with physicians.
Responsibilities include overseeing care, managing schedules, and participating in weekly case conferences. Requires active Texas RN license and 1+ year home health experience; EMR proficiency and valid driver's license expected.
