Job description
UW Health Care Direct is seeking an Occupational Therapist to empower Dodge County home health patients to participate more fully in daily tasks through skilled evaluation, planning and home-based therapy. The OT identifies patient needs and creates an optimizing plan for self-care during the home health episode.
Work schedules meet patient care needs; minimum 0.5 FTE with potential weekends/holidays. Licensure in Wisconsin and at least 1 year of OT experience preferred; CPR/BLS and reliable
