Job description
Alternate Solutions Health Network is seeking a Physical Therapist I to deliver skilled therapy in patients' homes as ordered by the attending physician. You will educate patients and families, document thoroughly, and ensure compliance with Medicare guidelines.
The role includes developing and adjusting the plan of care, coordinating with PTAs and aides, and driving to patient locations for scheduled visits.
