Job description
CenterWell Home Health is seeking a Home Health Physical Therapist to deliver personalized in-home physical therapy, improving strength, mobility, and independence for patients in their own homes. You will work under the Clinical Manager and collaborate with a multidisciplinary care team to optimize outcomes.
You will assess patients, develop rehab plans, oversee PTAs and Home Health Aides, and document progress while coordinating with physicians and families.
