21st Century Home Health Services Inc. is seeking a Full Time Physical Therapist Case Manager, Level 2 in the Castro Valley, CA area. This role involves evaluating patients in their homes and developing personalized care plans to assist them in regaining independence.
You will collaborate with PTA staff and communicate effectively with physicians to ensure high-quality patient care. Ideal candidates will possess a valid California Physical Therapist License and have strong critical thinking and interpersonal skills.
