Job description
TEEMA Solutions Group seeks a compassionate RN Care Manager to provide skilled nursing care to patients in their homes. The RN will manage an assigned caseload, conduct comprehensive assessments, and develop individualized care plans with the care team.
The role includes coordinating care with physicians and interdisciplinary teams, educating patients and caregivers, maintaining compliant documentation, and ensuring high-quality home health services. On-call rotation may be required.
