Job description
HCR Home Care is seeking an RN Case Manager to administer skilled nursing care to patients in the home, teaching families and supervising the nursing team as directed by the physician’s orders.
You will manage each patient’s care plan, coordinate with LPNs and Home Health Aides, conduct admissions, perform home visits, and document thoroughly to support positive patient outcomes and compliant operations.
