Job description
CenterWell Home Health in Hauppauge, NY, seeks a Home Health Registered Nurse to deliver personalized, one-on-one care in patients' homes. You will administer medications, perform wound care, conduct assessments, and help patients regain independence with support from physicians and clinicians.
You will develop individualized care plans, monitor patient progress, educate families, and coordinate with the care team.
