Job description
CenterWell Home Health seeks a Home Health Registered Nurse to provide one-on-one nursing care in patients' homes under the Clinical Manager. You will develop and implement individualized care plans, monitor progress, and collaborate with physicians to support recovery and independence.
Ideal candidates have Med Surg/ICU/ER or acute care experience, active RN licensure, and current CPR. Valid driver's license and reliable transportation required.
