Job description
Genesis Home Care in Ohio is seeking a Registered Nurse Case Manager to lead assessment, plan of care, and coordination of multi-disciplinary services in the home setting.
The RN educates patients and families, documents per Medicare guidelines, and ensures safety and quality of care across the patient’s episode, while maintaining licensure and compliance with regulatory requirements.
