Job description
RADIAS Health in Saint Paul, MN seeks a Homeless Outreach Mental Health Practitioner to provide PATH services to unsheltered individuals and PATH enrollees. You will maintain a caseload, collaborate with a multidisciplinary team, and coordinate housing and crisis services while engaging clients to achieve housing stability.
The role requires 6,000 hours of supervised experience and a background in homelessness services; travel in the community may be necessary.
