Job description
RADIAS Health seeks a Homeless Outreach Mental Health Practitioner to provide PATH services and direct care to individuals experiencing homelessness in Saint Paul. You will conduct outreach, screening, and case management to help clients set and reach recovery and housing goals.
A strong emphasis is placed on unsheltered individuals and coordination with the PATH team. The role requires a Bachelor’s degree in behavioral sciences, a valid driver’s license, car, and ability to transport clients;
