Job description
Signify Health, part of CVS Health, is hiring a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. The primary duties include conducting 45‑minute visits in members' homes, reviewing medications and medical history, performing a physical evaluation, and coordinating with health plans.
You will travel locally within licensed states, conduct virtual visits when needed, and provide Diagnostic & Preventive Services.
