Job description
Signify Health is seeking part-time clinicians (Nurse Practitioners or Physician Assistants) to provide in-home health evaluations. Clinicians will assess health status, review medications and medical history, and connect patients to appropriate care options in their homes.
The role includes local travel and occasional virtual visits to support health plans. The ideal candidate holds an active, unrestricted NP/PA license with board certification and is willing to travel within licensed states.
