Job description
Enhabit Home Health & Hospice in Rhode Island is seeking a licensed MSW to join the care team, administer social services to patients, and educate families as part of coordinated care. The role follows physician orders and the plan of care (POC).
The ideal candidate has at least one year of health care social work experience, current state licensure, and strong technology skills with mobile devices. Medicare home health or hospice experience is preferred.
