Job description
ACMC Family Home Health, part of LHC Group and UnitedHealth Group family, seeks an Occupational Therapist to join our home health team. You will assess and evaluate patient care needs related to functional status, ADLs, and home safety, and collaborate with the care team to develop therapy plans.
Responsibilities include performing patient assessments, documenting CMS-relevant status, and adapting care for diverse patients.
