Job description
Signify Health is seeking a part-time In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for adults in their homes. In this role, you will conduct detailed histories and physical exams, review medications, assess social determinants of health, and identify care gaps. Using mobile technology and EMR tools, you will document findings, support value-based care programs, and collaborate remotely with interdisciplinary teams.
