Job description
Alignment Health in California seeks a licensed Nurse Practitioner or Physician Assistant to perform in-home assessments, review medications, and conduct health screenings for senior members.
You will collaborate with Regional Medical Officers and care teams to create appropriate care plans, coordinate with PCPs and specialists, and guide a clinical team as needed. Active CA NP/PA licensure and EMR experience are preferred.
