Job description
Monogram Health seeks an Advanced Practice Provider (NP/PA) to deliver in-home, evidence-based care for patients with multiple chronic conditions. You will join an integrated team, perform comprehensive assessments, educate patients, and coordinate with PCPs and specialists.
