Job description
Monogram Health is seeking an Advanced Practice Provider (NP/PA) to deliver in-home, evidence-based care to patients with multiple chronic conditions. Join an integrated multidisciplinary team and visit patients at home 3-4 days per week, with telehealth visits and no on-call requirements.
You will perform assessments, manage medications, educate families, and coordinate care using EHRs such as Athena, Salesforce, and Epic.
