Job description
Lucet seeks a Provider to deliver in-home and community-based care across Polk County, FL, within a 50‑minute radius from your home. You will conduct visits, manage chronic conditions, perform assessments, and document in the EMR per regulatory standards.
Required are active licensure, FNP/AGNP/PA certification, DEA license and BLS, plus a clean driving record. Role may offer remote flexibility with compensation of $115,000–$140,000 per year.
