Job description
Signify Health is hiring an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive in-home health evaluations for vulnerable and chronically ill patients. You will conduct physical assessments, review medical histories, reconcile medications, and document findings in a technology-enabled platform. Collaborating with interdisciplinary teams, you’ll identify care gaps, support value-based care plans, and provide education that improves outcomes.
